COVID 19 Update July 2021


July 19th 2021 is when all restrictions for COVID are removed. The practice is a high risk environment and we are keen to maintain a safe and healthy place for you to be. Please help us to keep you and those around you safe.  Masks - the continued wearing of masks is current government guidance and is strongly recommended. Please wear your mask if you come into the practice for any reason.  Social distancing - we will continue to promote social distance awareness.  Please use chairs available in the waiting area, which are separated for distance purposes. If there is no seating left, please consider waiting outside until one becomes available.  Hand Sanitiser - this will continue to be provided. Please use when entering the building and when leaving.  Your help with keeping us safe is appreciated. Many thanks.

PreRegistrationIf you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. 

You will still need to visit the surgery  to sign the form to confirm that the details are correct. Please also bring the following into surgery to register.

 1.Medical Questionaire one for each family member ( found below)

2. Photo ID 

3.  NHS number if you have it.

4.  Proof of address if you have it.

NHS Counter Fraud and Security Management Services aim to reduce the number of people who claim NHS Care fraudulently. Please ensure you are entitled to NHS funded healthcare before registering to avoid any problems at a latter stage.

Pre-registration Form

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Please print off the registration form, complete it and return it to the Practice.  Please also bring with you photograph identification such as passport, id card, driving licence card.
